Your First Visit
While we enjoy working with new and returning patients alike, we especially enjoy meeting new patients and getting to know them during their very first appointment. For your convenience, you may print and fill out your new patient paperwork in the comfort of your own home. If you would rather fill it out in our office, please plan to arrive an additional 10-15 minutes early for your appointment. If you have dental insurance and are planning on using your benefits, please remember to bring your insurance card and a valid form of I.D. with you as well.
